What documentation do I need to get a handicap vehicle tag in Colorado?

In order to be eligible, you will need to have:

- A Colorado driver’s license, driver’s authorization, or temporary permit.

- A doctor’s statement.

- A completed handicapped parking permit application.

In addition, you must meet at least one of the following criteria:

* You have a permanent or temporary physical impairment that requires a permanent or temporary use of a mobility device.

* You have a permanent or temporary physical impairment that requires the assistance of another person to drive or get in and out of a motor vehicle.

* You have a permanent or temporary mental impairment that requires the assistance of another person to drive or get in and out of a motor vehicle.

* You have a permanent or temporary physical or mental impairment that results in you needing to sit in a wheelchair for mobility purposes.

* You have a severe visual impairment with a corrected Snellen visual acuity of no less than 20/200 in the better eye while wearing a corrective lens.

* You have impaired vision and your field of vision is restricted within a diameter of 20 degrees.

You will need to provide documentation that includes the following:

- Your full legal name

- Mailing address

- Physical address

- A phone number

- A clear and complete description of your impairment

- Certification by a Colorado licensed Physician’s Assistant, Advanced Practice Registered Nurse, and/or a Chiropractor that you meet at least one of the above criteria

- Any required attachments including:

* A mobility device order

* A written statement verifying your need for assistance from a third party

* A low-vision exam

- License Plate of Your Vehicle

All documentation must be on letterhead and dated.
