When did Chevy Silverado add third door?

The Chevrolet Silverado has always had four doors (two front, two rear) in its crew cab configuration. There was never a time it only had two doors and then *added* a third. The extended cab versions had smaller rear-access doors, but these weren't full-sized doors suitable for easy passenger entry/exit like the crew cab's rear doors.
