Fill in the required information on the form. This will include the names of both you and the cosigner, the make and model of the car, and the license plate number.
Sign the form in the presence of a notary public.
Take the form to the DMV. You will need to provide the following documents:
1. The original title to the car
2. A copy of your driver's license
3. A copy of the cosigner's driver's license
4. The completed Co-Signer Release Form
Pay the required fee. The fee will vary depending on your state of residence.
The DMV will process the form and remove the cosigner's name from the title. You will be given a new title that shows you as the sole owner of the car.
