Obtain the necessary tools and parts for the job. You will need a lug wrench, a flat-head screwdriver, a brake adjuster tool, and new brake pads if yours are worn out.
Engage the parking brake.
Loosen the lug nuts on the rear wheels to be adjusted, but don't remove them completely.
Raise the rear of the van using a floor jack or ramps. Never work under a vehicle supported only by a floor jack. Always use jack stands.
Locate the brake adjuster star wheel on the rear brakes. On the Grand Caravan, it's located just behind the brake rotor, inside the brake drum near the top.
Use the flat-head screwdriver to insert the brake adjuster tool into one of the slots in the star wheel.
Turn the brake adjuster tool to adjust the brake shoes as needed. Rotating it upward tightens the shoes closer together, while turning it downwards loosens them. Be careful not to over-tighten. As you turn, the brake shoes will press out and adjust until they lightly contact the brake rotor. Once light contact has been achieved, release any excess tension applied by turning the star wheel slightly back.
Repeat the adjustment for the other rear drum brake.
If you replaced the brake pads, it may take a few pumps of the brake pedal to seat the new pads properly.
Retract the parking brake.
Replace the lug nuts and lower the vehicle.
