1. Obtain the following forms from the Illinois Secretary of State's website:
- Application for Vehicle Title (Form VSD 190)
- Assignment Power of Attorney (if the person being added to the title is not present to sign)
2. Fill out and sign the forms:
>- Provide the vehicle identification number (VIN), make, model, and year of the vehicle
>- Enter the names and addresses of the current owner(s) and the new owner
>- If you are using an Assignment Power of Attorney, ensure both the current and new owners sign the form
3. Visit a Secretary of State (SOS) facility:
- Submit the completed forms
- Pay the appropriate fees ($35 title transfer fee and $2 for each additional owner name)
- Provide the original title and proof of insurance
4. Once your documents have been processed, the new title will be issued with all of the names listed.
Additional Information:
- If the vehicle is leased, you will need to contact your leasing company to determine the requirements for adding an additional owner.
- If you are adding an owner through a Power of Attorney, the individual must be present at the Secretary of State's facility to sign the title, or you must submit the notarized Power of Attorney form along with the other required documents.
- If you are adding a lienholder to the title, such as a bank or credit union, you will also need to provide the lienholder's name and address on the title application.
